We provide a full range to services to help treat our patients who have been diagnosed with a chronic illness including Asthma, COPD, Cardiovascular disease, and Diabetes among other chronic conditions. Our aim is to ensure you are receiving the best possible standard of care for your condition and improving your quality of life is of great importance to us.
As part of a new chronic disease management programme we now provide structured twice yearly check ups in certain qualifying conditions including those mentioned above for patients aged over 18 completely free of charge*. This involves both a nurse and a doctor visit and relevant investigations such as blood and urine tests and occasionally heart monitors (ECG). We will keep a record of when you are due your check-up and contact you to arrange this visit.
*The patient must have a Dr Visit card or Medical Card to avail of this service.
